Skype
Does exactly what it says it does and works through any firewalls that we have tested it with: www.skype.com.
It's also available for Mac/Linux/Palm and Windows.
Nice resource on Voice Over IP from my hosting company.
Get your head round their new system.

Skype is a great program. Its available on Windows right now but not available on mac, linux or palm YET.
